Abstract
With an ever-growing population of people with chronic diseases, it is estimated that Taiwan now has over 2.3 million diabetic patients. The crippling burden of diabetes on health expenditure is felt in healthcare institutions, large and small, and at governmental levels. At our hospital, a tertiary care medical center in central Taiwan, well over 10,000 patients are regularly treated for diabetes. With most of the patients enrolled in a diabetes shared-care program, it has been a constant challenge to deliver comprehensive care to the patients while complying with national health insurance reimbursement regulations and meeting the quality standards imposed by the shared-care program. Fortunately, with the full support of the hospital's administrative office, much resource and planning have been invested in diabetes case management at our hospital. Under constant supervision, various improvement measures have also been implemented to facilitate case management over the years. To further improve the diabetic patient care quality, CCH recently implemented Acer Healthcare VeriSee DR software for diabetic retinopathy. With Acer Healthcare's customization service, VeriSee DR was integrated into CCH's HIS and PACS. VeriSee DR provides AI-based diagnosis results in the background, and there is almost no change to our healthcare professionals' usual clinical workflow. After around six months of implementation, we found high consistency between the diagnosis results of VeriSee DR and the DM doctors. We are also starting to see positive impacts on ophthalmologists, patients, and the medical center as a whole. To face the growing burden of diabetes and the increasing complexity of its treatment and management will undoubtedly demand continual evolvement of the case management system.
